Taylor Made Order Form

If you would like to order using a check or money order, please complete the following form and print it. Send it to the address below with the check or money order enclosed. As soon as I receive the order and the payment, I will e-mail you the Download link and you can then print the play. Please do not send cash. I am requesting two e-mail addresses to ensure delivery of the link. Many times when e-mail is sent to addresses such as Hotmail, Yahoo, MSN, etc. the boxes fill up and the e-mail will not go through. The additional e-mail address is needed so I will have an alternate address to send it to.
